People with Crohn’s disease have trouble absorbing nutrients because the inflammation of the intestine makes it difficult for the organ to digest and absorb the nutrients from food. The patients low levels of sodium and potassium due to the lack of absorption could be what is causing her diminished sense of touch. Your nerves need a certain amount of potassium to control their function along with muscle function. Potassium and sodium help your nerves send electrical signals, via the sodium potassium pump, that are necessary for nervous system function. The patient's abnormal amounts of the two is causing irregular nerve function that is affecting her sense of touch and causing numbness.

The singular verb "is" does not appear to agree with the plural subject "amounts".  you would most likely want to change the "is" to "are" or "amounts" to "amount" to make the sentence smoother, and more correct. try reading your paragraphs out loud and maybe you can find these issues easier.

You could reword the first sentence to "Individuals affected by Crohn’s disease have trouble absorbing nutrients due to intestinal inflammation resulting in difficulty to digest and absorbing nutrients from foods." Overall, it was a concise paragraph.

The paragraph is concise and objective, however I think it would read better if the information was tied together in sentences such as "your nerves need certain levels of K and Na so that the sodium potassium pump can maintain membrane potential necessary for proper signal propogation". this may just be stylistic preference, but I perfer to read a single sentence that connects 2 ideas rather than 2 sentences as this feels choppy to me.
